What do you not like about health insurance providers past or present?

I'm a licensed health insurance agent and have been licensed in 45+ states and premium is a given - important to us all. However, every state given the ability to create legislation for their state, means we are still going to have too much of a variance in coverage state to state. For example, Indiana and Michigan passed legislation a few years back that insurance companies could not ride (exclude) coverage on any persons in their state with the intention the insurance company would have no choice but to cover. Instead, the insurance companies would then decline for everyday medical conditions. I think the real change has to be made where insurance companies cannot monopolize and cheat doctors from getting full rate or even half. Big insurance companies get a discount of 90% sometimes from negotiating where as uninsured people never negotiate costs and this is where insurance companies have a strong hold on doctors and control over the whole system.
I went to a lecture on The Cost of Health Care the other evening, it was very informational but I left with a headache literally, it's almost like studying for a college Calculus course all the various coverages and legislation.
I want to know how much it costs first and then what it covers.
there are also limitations based on what my employer offers...so i can only choose one of the three they offer, which aren't necessarily the best plans out there.
Basically, I want all of the choices in the survey. It's very important that my doctors be a participating provider, because I'm NOT going to someone else!

first i consider the cost and then the coverage
Oftentimes I find the coverage to not be worth the money we spend. They, insurance company, are always hunting for ways to get out of paying.
Right now I'm on Medicare and still pay out the butt for health coverage and prescriptions. Something needs to be done and soon.
I lke to know up-front what services are covered. If, for example, I am told I need surgery, what alternatives do I have?(e.g. physical therapy, going to a sports doctor, ). I would like to know if the insurance company will approve those as well, and how much they will pay.
